  10. Past Medical Treatment History (e.g., medications? high blood pressure? any operations? hernia? diabetes? kidney or liver disease? any treatment for drug or alcohol abuse?

  11. Disability History (any time off for injuries?  how frequent? how long? what injury or injuries involved? received any disability awards? etc.)

  12. Workers' Compensation Claims Activity (any past claims or pending claims?  what type? etc.)
